Transfer Drills --
​Strengthening Networks of Care Through 


We are stronger together!

Community Birth Transfer Drills use the Step Up Together approach which was built on high-fidelity simulations, designed by midwives, to improve how transfers of care occur between community birth settings, EMS, and hospitals. Their purpose remains to strengthen communication, clarify roles, and build practical readiness so that when a real transfer occurs, the handoff is timely, respectful, and clinically clear. These drills recognize transfers as a normal part of safe maternity care and focus on practicing coordination before urgency exists.
In Massachusetts, transfer drill implementation is coordinated by the Community Birth and Doula Initiative (CBDI). Through these drills and the work of CBDI, there will be resources made available to ensure coordination and continuity of care across settings statewide. CBDI is working to improve shared understanding of regional capabilities to ensure quality care for all birthing people.

Why CPM participation in transfer drills matters
(especially now)


Licensure makes CPMs visible within the system — transfer drills are where that visibility becomes operational, not just regulatory.


CPMs help define how transfers actually happen by bringing community birth workflows, documentation, and clinical judgment into the room.


Practicing handoffs builds credibility and trust with EMS and hospital teams before real urgency exists.


Clear, practiced transfers protect clients and midwives alike, reducing confusion, delays, and miscommunication.


Integration is learned by doing — CPM participation ensures licensure translates into functional, respected roles within the care system.
