The Bureau of Health Professions Licensure is holding open office hours to answer questions about the new LCPM advisory, temporary licensure, and the application process.

Virtual via Microsoft Teams
This is a good opportunity to bring questions about the temporary license application, required documents, MCSR, or anything else that remains unclear.

The licensure portal is now open!

What to Expect from the Temporary LCPM Application
The application is fairly straightforward, but gathering your documents first will make it easier. You can save your progress and return later.

Have These Items Ready:
  • A clear headshot
  • Your MEAC certificate or NARM Bridge Certificate
  • Information about current and past professional licenses
  • Your NPDB Self-Query report
  • A completed and notarized CORI form
  • Information about former names
  • Supporting documents for any history question answered “yes”

Education and NARM Verification
Upload your MEAC certificate of completion or NARM Bridge Certificate. MEAC graduates must have an official transcript sent directly by their school or records custodian.  Contact NARM at [email protected] and request verification of your current CPM credential. NARM must send it directly to [email protected]. You cannot submit the verification yourself.
Other Professional Licenses
List all professional licenses held in other states, jurisdictions, or countries, including expired licenses. Official verification must come directly from each licensing authority. Electronic verification can be sent to [email protected]. Mailed verification must arrive in an unopened, jurisdiction-sealed envelope.
The application does not ask you to upload another state’s laws or regulations.
NPDB Self-QueryRequest
Self-Query through the National Practitioner Data Bank and upload the complete, unmodified report to the application. This will incur a $3 fee
CORI Form
The CORI Acknowledgment Form must be printed, completed, notarized, scanned, and uploaded.
Make sure:
  • You enter only the last six digits of your Social Security number.
  • Your information matches your license application.
  • All former names are included.
  • You sign the form on the same date it is notarized.
  • The notary completes the entire authentication section.
Professional and Criminal History
The application asks about past or pending professional discipline, surrendered or denied licenses, and criminal history.
A “yes” answer requires an explanation and supporting documentation. A past matter does not automatically disqualify an applicant.
After Submission

You will receive an application number and a downloadable copy showing your application as pending.
The application may remain pending while the Board waits for documents sent by NARM, your school, or another licensing authority. Watch your email and the licensing portal for follow-up requests.

What to Expect from the MCSR Application

After submitting your midwifery license application, you will be directed to a separate Massachusetts Controlled Substances Registration application. The current fee is $300.
The application asks for:
  • Your contact and business information
  • Your principal practice location and mailing address
  • How you plan to use the MCSR
  • The drug schedule requested
  • Professional and criminal history information
  • An electronic signature
Your MCSR Address
Use your principal place of business or professional practice. If you do not have a separate business location, you may use your home address. The address will appear on the public license-verification website. A PO Box cannot be used without a street address. A separate MCSR is required for each location where you purchase and store prescription medications.
Medications You Plan to Stock
If you indicate that you will purchase and store prescription medications, MCSR may email you for a list. They want medications you expect to keep in stock, not medications you may prescribe without stocking.
You can reply directly to the email, and staff will add the list to your application.
The list is not binding. MCSR has confirmed that they only need a general idea of what you expect to stock. Your choices may change based on cost, shelf life, storage requirements, client volume, and the Board-approved formulary.
You may use this language if you'd like when listing the medications you plan on using in your practice:

"Below is the list of prescription medications I currently anticipate purchasing and storing for use in my clinical practice. My final purchasing decisions may change based on cost, storage requirements, expiration dates, client volume, and the formulary approved by the Board of Registration in Midwifery."

After submission, you will receive an application number, payment receipt, and downloadable copy of your application. Watch your email for follow-up requests from MCSR staff.

process Tips from our  community

— Rhode Island license holders, be prepared to fill out this form get a $50 money order/ certified check to send directly to RI in order to have them send a verification of licensure to MA.
*Fun Fact it can take 4-6 weeks for processing.

—NH license holders, be prepared to print and snail mail a form and a $15 check for license verification. Personal check appears to be sufficient.

—MLL Graduates can email [email protected] for help gathering transcripts. Others have said it may take some time but the persistent midwife is going to have a faster responses. In other words: Be the squeaky wheel.

—Transcripts are taking a long time to come from the midwifery schools. Remember to have transcripts sent directly to the BORM.

What to do when your midwifery schools has closed

For graduates of any other closed midwifery school:
  1. Contact the Department of Education or higher-education agency in the state where the school operated.
  2. Ask which agency or organization now holds the school’s student records.
  3. Request an official transcript sent directly to the Massachusetts Board of Registration in Midwifery.
  4. Ask about the fee, required identification, delivery options, and processing time.
  5. Include:
    • The school’s full name
    • Every name used while enrolled
    • Dates or years of attendance
    • Date of birth
    • Address used while attending
    • Current address, phone number, and email
    • The name and mailing address of the Massachusetts Board
  6. Keep copies of the request, payment, confirmation, and any correspondence.
  7. If the records cannot be located—or will not arrive before a licensing deadline—get that in writing and notify the Massachusetts Board promptly.
The key point is that records from a closed school are usually transferred to the state education agency or another designated records custodian. Applicants should not assume NARM’s copy will replace the official transcript unless the Massachusetts Board confirms that in writing.
For Birthwise Midwifery School:
  • The Maine Department of Education holds the records.
  • Email [email protected]
  • Subject: Transcript Request
  • Cost: $3 per copy, payable to Treasurer, State of Maine
  • Instructions: Maine DOE Transcript Request
  • Maine says requests may take five business days or longer.
  • Mailing address:
    Transcript Request
    Maine Department of Education
    23 State House Station
    Augusta, ME 04333-0023
    The request should include:
  • Every name used while enrolled
  • Dates or years of attendance
  • Date of birth
  • Address used while attending
  • Current contact information
  • Where the official transcript should be sent
